Service Description

The initial wellness consult will last one hour to gather health and wellness background information and explore recommendations. Payment Policy for Kina Wellness: Thank you for choosing Kina Wellness for your healthcare needs. To ensure a smooth and efficient experience for both our patients and staff, we have implemented the following payment policy for new patient appointments: 1. Payment Requirement: Payment for all new patient appointments is due at the time of booking. This includes the initial consultation fee and additional charges for services rendered during the appointment. If payment is received after the appointment, it must be rescheduled. 2. Accepted Payment Methods: We accept various forms of payment, including credit/debit cards, cash, and electronic funds transfer (EFT). Please ensure that your preferred payment method is readily available when booking. 3. Booking Confirmation: Your appointment will be confirmed upon receipt of payment. 4. Cancellation and Refund Policy: We understand that unforeseen circumstances may arise, causing the need to reschedule or cancel appointments. However, please note that payments for new patient appointments are non-refundable. If you need to reschedule your appointment, please get in touch with us at least 24 hours in advance to avoid forfeiture of payment. 5. Late Arrivals: We strive to maintain a punctual schedule to accommodate all our patients. If you arrive late for your appointment, please understand that your appointment time may be shortened to avoid inconvenience to other patients. However, the full appointment fee will still apply. 6. Insurance and Reimbursement: Please be advised that Kina Wellness does not directly bill insurance companies for new patient appointments. However, upon request, we can provide you with a detailed receipt containing all the necessary information to seek reimbursement from your insurance provider. 7. Additional Charges: Any additional services or treatments recommended during your appointment will incur additional charges. These charges will be discussed with you before implementation, and payment will be required before services are rendered. 8. Confidentiality: All payment information provided to Kina Wellness is strictly confidential and securely processed by industry standards. By booking a new patient appointment with Kina Wellness, you agree to abide by the terms outlined in this payment policy. Please message with any questions.


Cancellation Policy

24 hours are needed for cancellation. No substitutes for the lab included in the package; however, additional labs can be added for an additional charge. Program Overview The Wellness Program may include, but is not limited to: • Clinical or wellness consultations • Functional or conventional laboratory testing • Personalized wellness recommendations based on results • Follow-up visits or check-ins as outlined in the selected program Specific services, visit length, and laboratory panels are defined at the time of purchase and may vary by program tier. Purchase & Payment Terms • Full payment or agreed payment is required prior to program initiation. • Enrollment is confirmed only after payment has been successfully processed. • Prices reflect the bundled cost of professional services and laboratory testing unless otherwise stated. • Promotional pricing or discounts cannot be applied retroactively. Laboratory Testing Terms • Laboratory tests are ordered based on clinical or wellness discretion and program scope. • Lab fees may be: o Included in the program price, or o Billed separately, as clearly disclosed before purchase. • Once laboratory tests are ordered or specimens collected, lab-related fees are non-refundable, as labs are processed by third-party vendors. • The client is responsible for completing labs within the timeframe provided to ensure accurate interpretation and continuity of care. Insurance & Reimbursement • Wellness programs and specialty labs are typically not covered by insurance. • The client is solely responsible for any costs not covered by insurance, including denied claims. • Upon request, a receipt may be provided for potential submission to HSAs, FSAs, or insurance carriers; reimbursement is not guaranteed. Cancellations & Refunds • Program purchases are non-refundable once services have begun or labs have been ordered. • If cancellation occurs prior to any labs being ordered and before the first appointment, a partial refund may be issued at the discretion of the practice, minus an administrative fee. • Missed appointments or failure to complete labs does not qualify for a refund. Rescheduling & Program Modifications • Appointments must be rescheduled in accordance with the practice’s scheduling policy. • Program components cannot be substituted or transferred to another individual. • The practice reserves the right to modify program structure or lab vendors as clinically appropriate, without reducing overall program value.
